Interview Documents

At the time of their interview, applicants will need to provide the following documents:

  • Valid passport.
  • (2) Frontal color photos measuring 50mm x 50mm, with white background.
  • Birth certificate.
  • Current marriage certificate (if applicable).
  • Divorce certificates from any previous marriage (if applicable).
  • If widowed, death certificate from previous spouse.
  • Documents that prove the relationship between the CFRP applicant and the petitioner. In the case of a brother or sister, the applicant will need to provide his or her birth certificate and that of the petitioner.
  • Police certificates/court records (for applicants sixteen years of age or older) issued within six months of the visa interview date.
  • Medical exam. Applicants must follow instructions on the interview appointment notice to have their medical examination performed at Hospital Manuel Fajardo located at C Street No. 720 between Zapata and 29, Plaza de la Revolución, Vedado, La Havana. Please keep in mind that you may wish to have the medical examination for derivative children done ONLY IF the children are included in the case AND they will be under 21 years old and unmarried at the time of their scheduled interview.  Derivative children who are 21 or older or are married on the day of interview will not be able to remain in your case and so will not need to submit a medical check or other document.
  • Affidavit of Support, Form I-134 (PDF 100 KB)  (For CFRP applicants accepted into the CFRP program before February 17, 2015 only if expressly requested by a consular or DHS officer).
